Appointments are subject to provider availability and confirmation by Elevate Spine & Health.
At this time, appointment requests are manually reviewed and scheduled by our office.
5. Cancellation & No-Show Policy
Elevate Spine & Health reserves the right to implement cancellation, late cancellation, rescheduling, or no-show fees at its discretion.
Patients may be charged fees for missed appointments or appointments canceled without reasonable notice, including cancellations made less than 24 hours before a scheduled appointment.
Current policies, fees, and scheduling requirements may be updated or modified at any time without prior notice.
6. Payments, Pricing & Financial Responsibility
Payment arrangements, memberships, treatment plans, pricing structures, and service fees may vary depending on services provided.
Payment may be due at the time of service unless alternative arrangements have been agreed upon by the office.
Prices, membership plans, promotions, and fees are subject to change without notice.
Elevate Spine & Health does not guarantee insurance reimbursement. Patients are solely responsible for understanding their own insurance coverage, out-of-network benefits, and reimbursement eligibility.
